RN Field Assessor:/h5:
Up to 75 per assessment
The Field Assessor assists our Health and Insurance Organizations in gathering information to complete related forms, reports and at times clinical assessments.
?
The Per:Diem Field Assessor will participate in a variety of responsibilities as dictated by their scope of practice of their clinical license.

Time?

The positions starts as Per Diem and depending on the case load demand in the area, moves to part:time or full:time. Our interviews and assessments may average 1:2 hours each and be located within a 50 mile radius of your home location.

Compensation?

We compensate up to 75 in some scenarios per in:person visit, less for telephonic interviews and compensate for most situations of no:shows (Detailed fee schedule is provided at hire). We offer direct deposit on a bi:weekly basis.

Credentials:

Licensed Registered Nurse must have at least one year of post graduate relevant case management experience.

Required experience:

Complex Care Management/Case Management/Managed Care experience
:
Community based or In:Home visit experience
:
Experience working with either Geriatric, chronically ill or functionally challenged populations

Computer skills and resources:
?
:
Ability to use a variety of electronic information applications/software programs, electronic medical records experience
:
Intermediate to Advanced computer skills and proficiency with Microsoft Word, Outlook, and Excel, excellent keyboard and web navigation skills
:
Access to a secure computer in order to accommodate data entry within required timelines
:
High speed internet connectivity (DSL or cable modem; recommended speed is 10Mx1M for optimal performance)

Mobile phone, Driver's license, car insurance, and access to an automobile for home visits to member is required

Job Description?

Field Assessment and Reporting services will help plan members:
?
:
Accurately relay needed member information to their plan holder.
:
Assess Member and/or family needs for psycho/social/functional status in the location in which the elder resides and documentation in required formats by contractor and agency
:
Timely Assessment of Health Screenings and documentation in formats as required by contractor
:
Design a plan of care appropriate to the needs of the elderly/disabled Member and the family
:
Implement the plan of services as agreed to by the Member
:
Communicate among providers from multiple settings re: Members plan of care
:
Educate caregivers re: special needs
:
Access community based services and entitlements for Members
:
Assess appropriateness of the home environment and its impact on the plan of care
:
Provide reports of services rendered to Members
:
All other services related to care management as are reasonably requested by Agency, Member, Members responsible parties


Responsibilities:

Identify risks, gaps in care and prevent unnecessary hospitalizations and emergency room visits by developing an individualized interdisciplinary plan of care for members and families to follow in order to achieve lifelong well:being in the home

Comply with all on:boarding, annual and other mandatory training as assigned.

Collaborate with other disciplines
:
Maintain HIPPA compliance
