Our client is seeking a Board Certified Family Medicine physician to provide services at a new health center located onsite Corporate campus in the San Francisco Bay Area; this patient centered medical home is scheduled to open in September. Employed position with highly competitive salary (mid $190K) with bonus structure based on quality/prevention/wellness; outpatient only; 12-16 patients day; four weeks vacation including one week CME; full benefits; post residency practice experience preferred. The schedule at this outpatient clinic will be Monday to Friday daytime hours (9AM to 6PM or 8AM to 5PM) and may include some non-traditional hours to accommodate patient accessibility such as on call availability via cell/pager on weekends.
Key responsibilities will include:
* providing comprehensive clinical patient care through interviewing, examining and treating employees and their dependents in order to meet their health and wellness needs
* exercise final medical judgment in all issue of health care and determines level of urgency of follow-up, referral/consultation appointments
* prescribes appropriate medical treatment and medications for patients and orders studies, tests and ancillary care services as required
* refers to appropriate specialists outside the onsite clinic in a medically appropriate and efficient manner
* follows established rules and guidelines of evidenced-based care and best clinical practice
* practices in a collaborative fashion consistent with the concept of patient centered medical home
* refers patients to and utilizes unique internal employee benefits and resources to maximize health outcomes
*documents all services in an electronic patient medical record
* regularly reviews chronic medical conditions that require ongoing medical attention (educates patients on wellness, prevention and early detection by providing materials and resources to the patients and families
* demonstrates exceptional and effective collaborative partnership and communication style with both patients and staff
* consults with medical director as needed regarding patient care, assessment and education issues; participates in quality improvement, management
continuing education and other patient care programs established by the clinic requirements for organization and Safeway, Inc
* maintains privacy and confidentiality of all patient information according to federal and state guidelines and regulations
* effectively communicates with patients through technology to include email, video, telephonic and text
* uses telemedicine (via MDLivecare) technology to remotely treat patients who do not require an in person visit
