Institute on Aging (\"IOA\") is a San Francisco based non-profit agency dedicated to preserving the dignity, independence and well-being of aging adults and people living with disabilities. SPECIFIC RESPONSIBILITIES:

* Provides nursing care for participants at the Day Health Center.

* Administers oral medications, subcutaneous and intramuscular injections as prescribed by IOA/On Lok primary care providers.

* Communicates with pharmacy, clients and families to arrange for delivery of medication to client homes.

* Maintains all medications orders and other medication related documentation.

* Communicates medication orders to appropriate team members regarding medication changes and updates.

* Communicates medication compliance to appropriate team members and pharmacist.

* Communicates changes in medication orders to clients, families and caregivers.

* Obtains laboratory specimens per IOA/On Lok primary care provider order, including collection of urine, stool, sputum, wound cultures, blood samples.

* Performs preventative and rehabilitative nursing procedures appropriate for the participant's care and safety, such as vital signs, wound care, nebulizer treatments, urinary catheterizations, tube feedings, and ear washes.

* Responsible for completing all relevant documentation, including charting in medical records, utilizing a computerized system, and keeping statistical records.

* Functions as part of an interdisciplinary Attends and actively participates in team meetings.

* Actively participates in and encourages actions that promote good public relations and education on medication management with participants, their families and friends, visitors and the community.

* Participates in QI activities, such as reduction and prevention of medication errors and promotion of medication compliance.

* All other reasonably related jobs as assigned.

DESIRED QUALIFICATIONS

* Acute care and Geriatric experience preferred.

* Prefer experience common in a clinic setting, such as wound care, urinary catheterizations, etc.

* Ability to work flexibly within a multidisciplinary team and with a multicultural population.

* Evidence of creativity, flexibility and professionalism.

* Computer literacy preferred.

* Bilingual (Chinese/English, Spanish/English preferred).

EDUCATION:

* Graduate of an accredited Licensed Vocational Nurse program, with a current LVN license in California required

PHYSICAL REQUIREMENTS:

* Ability to lift up to lift, transfer, push/pull, maneuver, and reposition 35lbs.

* Ability to reach, bend and walk.

* Five finger manipulation.

ADDITIONAL REQUIREMENTS:

* Ability to comprehend complex written material and verbal instructions.
